As per this dnd 5e spell you do touch the inflicts disease. You can easily make a melee spell attack which is against a creature within your reach. On a hit, the target has been poisoned.

Al though, at end of each of the poisoned target’s turns, the target should make a Constitution saving throw. Suppose, if a target succeeds on three of these saves, then it is not longer be poisoned, and after that spell ends.  Anyway if the target fails three of these saves and the target is no longer been poisoned, but select one one of the diseases below. The target has been subjected to the selected disease for a spell’s duration.

However, since this spell has been induce a natural disease in its target, and any effect which may removes a disease or otherwise ameliorates a disease’s effects apply to it.

Blinding Sickness: As per this effect the pain grips a creature’s mind, and also its eyes turn to milky white. In this manner the creature has disadvantage on the wisdom checks and even the wisdom saving throw and is blinded.

Filth Fever: Here a raging fever shall sweep via the creature’s body. Of course the creature has the disadvantage on the strength checks, the strength saving throws, and also the attack rolls which use Strength.

Flesh Rot: Because of this disease the creature’s flesh decays. However with this also the creature has the disadvantage on Charisma checks and even it is vulnerability to all the damages.

Mindfire: Here all of the creature’s mind shall become feverish.  The creature has some disadvantage on an Intelligence checks and on the Intelligence saving throws too, and even the creature shall behave as if it is under an effect of the confusion spell during the combat.

Seizure: As per this the creature is overcome with shaking. Usually, the creature has some disadvantage on the Dexterity checks, Dexterity saving throws, and also the attack rolls which uses the Dexterity.

Slimy Doom: Here always the creature begins to bleed uncontrollably, the creature has disadvantage on the Constitution checks and even the Constitution saving throws. In an addition, whenever the creature takes the damage, it is almost stunned until an end of its next turn.
